Subway "Free Cookie" Survey Codes
Summary
Subway issues codes for a free cookie as a thank-you for completing a
survey. The codes are supposed to be used at the same restaurant from
which the original purchase was made, but this doesn't seem to be
closely watched.
The survey codes themselves seem to be very simple:
10-[Store Number]-12
The store number is printed on the top of your Subway receipt. So if
you bought food at store #1234, the survey code would appear to be:
10-1234-12
